- Each student’s birth certificateA full-color photo with all four corners showing.
- A household bill from the last 60 daysElectric, water, gas, trash/sewer, landline phone, internet, or cable.
- Your Arizona home addressThe physical address where your family lives.
- Your phone’s cameraYou’ll take the photos right inside the form.
Special situations are covered
- PO Box only? Upload a physical address letter from your fire or police department.
- Reservation address? Upload an address letter from your tribal agency or Chapter House.
- In the ACP? Upload a photo of your Address Confidentiality Program card.
